I thought it only goes to Goal Difference if all results (in the head-to-head-to-head) between the three are tied.

Think of how generally poor Italian sides have been in the Champions League and how long it took them to actually lose their fourth place (from this season only). Plus, they'd still get coefficient points in the Europa League (as would Stoke and Fulham).
